Clinical Pharmacist

Job Summary Evaluates and interprets physician orders and dispenses medication and other pharmaceutical supplies in accordance with established operational procedures, professional standards or practice. Provides pharmaceutical care to assigned patients under the general supervision of the clinical pharmacy coordinator. Participates autonomously in clinical rounds and routine monitoring of the drug therapy of assigned patients. May provide in-service/educational presentations to pharmacists, prescribers, and nursing. Minimum Qualifications Education/Training Pharm D. from accredited college/university Experience Completion of a pharmacy practice residency OR at least three years of relevant experience License/Certification/Registration Maryland Pharmacy license Knowledge, Skills & Abilities Ability to communicate effectively with all internal and external customers. HIPAA polices on Confidentiality Primary Duties and Responsibilities Preparation:
Prepares and dispense medications and IV solutions as order by physician in an ethical and legal manner. Accurately dispenses additives for IVs, TPN's, and irrigating solutions. Mixes or supervises the mixing of IV additive solutions and chemotherapy as required using aseptic technique. Accurately interprets physicians' orders enters them into the computer in a timely manner. Accurately reviews physician-orders ensuring that appropriate doses and medications have been prescribed, and that incompatibilities are handled appropriately. Determines that ordered medications are prepared and dispensed to the correct patient. Maintains inventory and records on narcotics and other forms of drugs as required by law. Monitors drug therapy as prescribed by medical staff to ensure safety and appropriateness of use (i.e., dose, allergy diagnosis). Clinical
Responsibilities:
Participates autonomously in patient care rounds. Reviews patient specific data to evaluate patient's status and medication needs and develops a therapeutic plan. Assures compliance of prescribers with clinical practice guidelines, minimizes nonformulary drug use, eliminates therapeutic duplication and makes recommendations for appropriate Monitors drug therapy of assigned patients. Identifies areas for optimum drug therapy and assures drugs/dosages are accurate based on the therapeutic intent and the patients' physiologic functions. Reviews patients' profile to determine possible contraindications with the prescription. Complete drug use evaluations, monograph reviews and the development of protocols and clinical practice guidelines under the guidance of the Clinical Pharmacy Coordinator Supervision:
Supervises Pharmacy technicians filling prescriptions. Supervises the prescription filling practices of pharmacy technicians and assists them in whatever is necessary. Carefully reviews orders filled by technicians to ensure accuracy. Supervises the mixing of IV additive solutions and chemotherapy as required using aseptic technique. Education:
Provides education to patients and medical staff on prescribed medications. Provides medication counseling to patients and medical staff. Provides drug information. Provides product information regarding products, product availability, use, dose--and adverse affects, to patients, physicians and nurses to assure safe and effective utilization of meds and IVs. Develops and conducts in-services for nurses, technicians and pharmacists. Development:
Maintains current knowledge, certification, and licensure. Identifies learning needs in self and initiates actions to address these education needs. Attends continuing education programs in order to maintain certification. Attends hospital sponsored training programs as required by the department. d. Maintains certifications. Organizational knowledge:
Participates on inter-departmental teams and committees, communicating unit knowledge as appropriate. Provides department/unit with information and knowledge acquired during participation with interdepartmental teams and committees. Demonstrates a constructive approach during all interactions with staff, supervisors, and managers both inside and outside the unit. Department/Unit Specific Functions and Duties Pitches in to cover shortages within department. Checks 24-hour exchange carts. Checks updates. Orders needed IV supplies and puts order away. Puts daily order away. Completes floor stock inspections. Salary Range:
$80K -- $100K

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
